1. Employee can choose from $40,000 to $250,000 in benefits. If an employee enrolls, their spouse/domestic partner can also enroll.
Spouse/domestic partner rates are based on their own age and tobacco use status.

Non-working spouse/partner can choose from $40,000 to $50,000 in benefits.
